Membership Application

Join by On-Line Payment or Mail-in Application!

Membership Period: January 1, 2011 through December 31, 2011

Minimum age is 14 years of age. Members under 18 years of age will need parent's or legal guardian signature to join CSO. Any member under age 16 must be accompanied by parent or legal guardian on all rides.

Application Instructions for On-Line Payment Method

• Read Code of Conduct & RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AND PARENTAL CONSENT AGREEMENT ("AGREEMENT") Click here to read
• Complete Application Form and press "Submit".  This will direct you to PayPal for payment.  You do not need a Paypal account.  You can use your credit or debit card.
• If you do not complete your paymet with Paypal, your membership will not process.
• You should receive an acknowledgement by email that your membership purchase was successful.  If not, please contact Carl.
